Uncovering the deeper causes behind sentinel events in healthcare—often rooted in organizational culture. We go beyond revising policies; we analyze systemic patterns and highlight risky behavior that interferes with safety and quality of care.
At KHDR Consulting, we uncover the underlying causes of sentinel events—many of which are deeply rooted in organizational culture. Rather than simply revising policies, we analyze systemic patterns and identify high-risk behaviors that compromise safety and quality of care. Our approach leads to more engaged stakeholders and greater overall satisfaction across the organization.
Therefore, we encourage everybody on the team to speak up as an equal member and voice concerns without fear of retribution. This process is critical to decrease the odds of future adverse events.
We encourage deeper relationships within teams and across teams to increase connection, collaboration, and compassion.
We engage patients and families as partners from the outset, ensuring a comprehensive investigation.
We assess contributory factors across the patient journey, examining longer time periods for a deeper understanding of what really occurred.
We do not just address failures; we study successes and recovery processes to promote resilient healthcare practices.
By evaluating clinical processes and their deviations, we provide structured recommendations for sustainable improvement.
Instead of checklist-driven fixes, we foster a culture where safety is ingrained in teamwork and everyday practices.
Our workshops empower leadership to enact meaningful cultural shifts, reducing the need for reactive policies.
We deliver tailored consulting, coaching, and training solutions for organizations, teams, and individuals. Grounded in real-world expertise, our approach drives meaningful change across healthcare and leadership. Our impact is focused in the following key areas:
